To Register as a New Patient

To register as a patient you will need to complete a new patient registration form, these are available in different formats (Website Form, Online NHS Patient Registration Service or contact reception for a paper form which can be collected).
Don't forget to register for Online Access to your medical records, prescriptions, appointments (Please see form below)
Whever possible please complete all patient registrations are online, these are processed immediately.
To ensure the registration process is completed smoothly and efficiently
- Please provide your NHS number- this helps identify you.
- The name and address of your last GP - this helps locate your Medical Records
- Any child health records you hold for children under 5 years of age.
- If you have recently come from abroad – the date you arrived in the UK and any previous addresses or previous GP details
- Please complete the New Patient Registration Form in full
- Remember to include your mobile number and email address as we send letters and appointment invites/ reminders via to mobile numbers or email addresses
At the time of registration you have the right to express a preference for the GP who you wish to be registered with. However, we are a group practice and you will be advised to see any available GP when requesting a consultation.

Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Disabled Patient Facilities
Our current facilities include:
- easy access via automatic doors
- all consulting rooms are wheelchair accessible
- a lift so wheelchair bound patients can now be seen on either floor
- a lower level reception desk to enable better communication with wheelchair users
- disabled toilets on each floor
